Goal Tracking for Social Services
If you need to track Clients’ milestones - like completing education, improving their mental health, or gaining stable housing - then Collaborate’s Goal Plans are for you. This feature allows you to:
- Ensure consistent data collection with templated Plans, Goals, and Steps,
- Measure progress toward Goals while adding narrative context, and
- Report the impact of Goal-centric programs.
Step-Based Progress
Typically, Goals are broken up into Steps to be completed. If a Client wants to earn a GED, for example, they’ll need to meet requirements, attend classes, and eventually pass the GED tests. Along the way, Staff can view at-a-glance progress updates in Collaborate.
When a Client completes a new leg of their journey, Steps can be marked as “Achieved” with a single click.
Goals in Context
With Goal Plan Reviews, Staff can add narratives around Client progress, such as reasons why they might be stuck on a certain Step - and what the solution is.
For additional context, Goals can also have Notes & Documents attached to them.
Visual Goal Reporting
All aspects of Plans, Goals, & Steps are measurable with Collaborate’s Reports. At a high level, you can use charts see what percentage of total Goals have been achieved.
Then, dig deeper to see how individual Goal Steps across your client base are progressing.
